To send us a periodontal assessment of your patient, you use our online form (see a completed example). We would prefer if you took advantage of the online form available, but, if this is not appropriate, please consult with us by email.

The online form need not be completed in one session. Once you have completed the first page of the patient's details, you can return to where you left off at any time. As each part of the form is submiited, it is automatically saved. The form can be printed out for your records; the comment areas can be filled by copy and paste from a word-processor. Other documents may be sent as supporting evidence.

Notes on the use of the form

The form is a comprehensive record of your patient's oral status, taking into account many factors that might contribute to oral conditions.

Many of the questions asked may not in fact apply to your patient, and if this is the case, please leave blank the questions that you cannot answer, or that do not apply, or preferably annotate them as n/a (ie not applicable).

It is important to give as much information as possible so that we can give you the best possible advice on how best to manage your patient's problem.

It would be extremely useful if you could send us photographs and/or x-ray images of your patient. These can be forwarded as an attachment to an email to services@periodontal-diagnosis.com, preferably in jpg, gif or bmp format. Alternatively, these could be sent to us by surface mail but you need to contact us to make the necessary arrangements in advance.

Always include your invoice number in any communication (the number is issued when the assessment form is ordered on-line).
